    Member’s wishes

    This relates to the lump sum death in service payment. It is used to guide the trustees as to who you would like to nominate to receive the lump sum payment.

  • Dependent nomination

    If you are not married, this form can be used to nominate an adult dependent who is financially dependent on you, to receive a pension on your death.

  • Query form

    If you have a query regarding your membership details, such as needing a retirement quote.

  • AVC enquiry

    For an AVC quote

  • Transfer in enquiry

    If you have previous pension benefits and would like to enquire about transferring these benefits to PAS.
