In biomedical observational research, evidence of underlying association between an exposure and subsequent disease is difficult to demonstrate unequivocally. Evidence of causation may only be inferred, yet there are many real-life situations for which we seek an understanding that cannot be subjected to experimentation. Epidemiology, the methodological foundation of observational research, provides structure to the conduct of biomedical research through appropriate study design, conduct, and statistical analysis. However, many study designs experience real analytical challenges and overcoming these is an ongoing mission for applied methodological researchers such as myself. In my main area of interest - lifecourse epidemiology - one studies adverse events in later life in relation to exposures that may have occurred throughout the lifecourse, possibly before birth and during pregnancy. Moreover, it is the accumulative compounding experiences of many exposures throughout life from conception, birth, and early childhood, through adolescence, early adulthood and older age, that one has to disentangle in the search for putative factors that affect health and well-being and which may lead to disease and morbidity or ultimately death. My research seeks new analytical strategies for these types of observational problems within epidemiology. I therefore focus on the development of robust statistical methodology and the appropriate application of statistical modelling to examine observational (often cohort) data, to assess if and how potential experiences throughout the lifecourse might lead to adverse health outcomes later in life. I examine human growth and seek to establish appropriate analytical strategies to inform research questions in relation to a variety of health outcomes (cardiovascular disease, cancer, childhood obesity, anxiety, depression, to name but a few).
Recent Projects
Methods for modelling repeated measures in a lifecourse framework Medical Research Council, UK Apr 11 - Mar 14 £415,439 (joint with Bristol University; £74,889 to Leeds). Answering questions about the relative importance of aspects and timing of growth, behaviour and health status for longer-term outcomes requires longitudinal data, but there are many challenges analysing these data by standard regression models. This project focuses on developing more robust methods for the analysis of such data, addressing issues around detecting and modelling autocorrelation, lack of constant variance within growth trajectories, and the selection of a best-fitting parameterisation of growth curves. We use our new methods to answer scientifically important questions pertaining to ALSPAC and data from other longitudinal studies available to co-applicants.
Leukaemia and other cancers in teenagers and young adults in England: an aetiological analysis applying new statistical approaches Children with Leukaemia, Apr 09 - Mar 12 £120,351. This project addresses the paucity of information on the causes of teenage and young adult leukaemia, lymphoma and central nervous system tumours by carrying out a programme of aetiological research focusing on infections. The work exploits the national cancer dataset available from the North West Cancer Intelligence Service for 7,000 individuals aged 15-24 years, diagnosed 1996-2006. Structural equation modelling and latent variable analyses will be used to investigate the timing of infection using a validated surrogate measure, providing new insights into the causes of cancer diagnosed in teenagers and young adults. This study will test the hypothesis of an infectious aetiology.
EnviroGenoMarkers: Genomics biomarkers of environmental health European Framework 7, Brussels Mar 09 – Feb 12 (joint with 10 partners; €204,000 to Leeds). This proposal is the first large-scale application of the whole range of ‘omics’ technologies in a population study aiming at: (a) the discovery and validation of novel biomarkers predictive of increased risks of chronic diseases in which the environment may play an important role (breast cancer, non-Hodgkin’s lymphoma, allergy, neurological and immune diseases, thyroid disruption); (b) the exploration of the association of such risk biomarkers with environmental exposures, including high-priority pollutants (carcinogens and immunetoxicants such as PCBs and PAHs, neurotoxicants such as cadmium, lead and ambient air pollution) and emerging exposures (such as phthalates and brominated flame retardants), many of which are also endocrine disruptors; and (c) the discovery and validation of biomarkers of exposure to the above and other high-priority environmental exposures (e.g. water disinfection by products). Leeds is the data warehouse and provides statistical bioinformatics expertise in the discovery and subsequent analyses stages.
Improved statistical modelling for observational research: understanding and addressing the problems associated with collinearity Medical Research Council, UK Oct 08 - Sep 12 £89,000 Statistical models with correlated covariates suffer collinearity. It is essential to distinguish between the positive (confounder adjusted) and adverse (biased) effects of collinearity. This project will distinguish between positive and negative impacts of collinearity and generate algorithms to assist systematically in the model building process to provide minimally biased estimates observational studies.
