There are several ways you can notify us if you have changed your Leeds address, mobile or house telephone number

Please select your preferred method from those below, but if you have changed address please ensure it remains in our practice area: University Accommodation in Leeds OR private accommodation in postal areas LS1 to LS7 or LS16 South of the Ring Road. 	INTERNET - Click here to open our web form
	E-MAIL - Send an e-mail to lsmp.address@nhs.net Include: Your full name and date of birth (must be date of birth not University ID number) Tell us your old LEEDS address, mobile and home telephone number Tell us your new LEEDS address, mobile and home telephone number We will e-mail an acknowledgement of receipt
	TELEPHONE - Monday to Friday between 10am and 6pm you can telephone 0113 295 4488 and ask the Receptionist to update your records
	COME IN to our reception and ask for a change of address form. Reception is open Monday to Friday, 8.30am to 6pm

If you have changed address and have been referred to a hospital, make sure you inform them too or you will miss your appointment - you can contact our Medical Secretaries for help with this
