This web site is designed for use by current and prospective patients of LSMP. Other users should bear this in mind when interpreting the information displayed on the site pages. If you are not a patient of the practice please take any medical queries arising from your use of this site or link pages to your own doctor. The aim of the site is to provide a wide and up to date source of practice and medical information. Any suggestions for improvements are welcome

WEB SITE ACCESS FOR DISABLED PERSONS
In pursuance of the requirements of the Disability Discrimination Act (1995) and the Special Educational Needs and Disability Act (effective from September 2002), the University of Leeds (which hosts our site) is obliged to make all its web sites (and those which it hosts) accessible to disabled persons. Following discussions the University has formally adopted the World Wide Web Consortium's Double-A standard as the norm for the University's web sites. Accordingly this site meets the same standards

  1. Purpose of this Statement
    This statement tells you how Leeds Student Medical Practice will collect and process your personal data when you access this web site
  2. Automated Collection of Personal Information
    As with most other web servers, when you access these web pages certain information you provide will automatically be recorded by the University, who provides our web hosting. This personal information will include your IP address, browser type, and information relating to the page you last visited. This information is processed to estimate how much usage of the server is made by different categories of users and in the event of a breach of security may be used to aid detection
  3. Non-Automated Collection
    At this time we do not collect any personal data. If we did we would write here the purpose of collection
  4. Third Party Access
    If you provided personal data it would not be sent to other third parties and would remain confidential. It would by accessed only by those who have a legitimate need to know within Leeds Student Medical Practice

    This web site does not use cookies, but you should note that the registration site that we connect you to when registering as a new patient uses cookies to facilitate the registration process

DATA PROTECTION NOTICE
Patients of Leeds Student Medical Practice should be aware that we store your data on computer

  • In certain circumstances non-medical personnel in the practice as part of their administrative work view this data. Our personnel are trained in the principles of confidentiality. They all sign a legal contract agreeing to maintain confidentiality
  • Data is given to Leeds Health Authority to enable financial claims and to Child Health to maintain their vaccination and developmental check records. Personnel there are bound by the same principles of confidentiality
  • Data may be provided to secondary care providers (consultants, hospital departments) during the referral process, and to tertiary care providers (social services). Your approval will be obtained before this data is provided
  • Data stored on the computer is used for audit, statistics, research, and planning. If someone outside the practice requires it, we provide data in an anonymised format, or seek your permission before disclosure of identifiable data
  • For medico-legal purposes we retain your data after you leave this practice
  • The practice has appointed a Caldicott guardian who ensures confidentiality is maintained, and that all data is stored safely using security procedures to limit access

SITE HISTORY

  • The site was first published in August 1999
  • In March 2002 it was replaced by a completely new site, with a fresh design
  • In April 2004 the design was restructured (frames were replaced by templates), and several new sections were created
  • In July 2005 the site was completely reviewed, redundant sections were removed, and the Health Advice and News pages were significantly re-designed
  • In May 2006 new menu buttons were introduced, the page width was reduced to 80% in-line with 'fashionable' web design, and the home page was redesigned to remove some JavaScript (which caused Internet Explorer security panel to flag it as a security risk for some PCs)
  • In September 2006 the home page was redesigned to reflect our new corporate image, new logo was introduced to all pages, on-page menus were replaced with text links instead of buttons
  • The content continues to be checked to ensure it demonstrates current best practise
