Sick notes for University departments
Sick notes to departments are not routinely required for absences of
less than seven days (unless you miss an exam or assessed work deadline
as a result of illness)
It is essential that you inform your tutor as soon as possible about any illness especially if near an examination or assessed work deadline
Many University of Leeds schools/departments will let you self-certify for the first 7 days using the University of Leeds Student Portal and selecting: Student Services & Admin | Log me into student services | Student services | Absence notification
If your tutor/department subsequently requests a a doctors written notification of illness, or sick note, please fill in a Consent for
Release of Medical Details form at Reception. Obviously for us to provide evidence of your illness you must have seen one of our
doctors or nurses at least once during your illness. You may also wish to read our Exams

SICK NOTES FOR EMPLOYERS
Similar parameters apply for sick notes required by employers:
For the first seven days of illness you need to complete form SC1 (available
from your employer) thereafter we will give you a sick note
If your employer requires an additional sick note from us for the first
7 days they will need to pay for a private certificate - contact Reception
for current charge. For us to give you a private certificate you must
have been seen by a doctor or nurse at least once during your illness
