Registration

We offer five methods of registering with Leeds Student Medical Practice. Follow the guidance on this page and choose your registration option

A note for new students - the University of Leeds will supply you with one of our medical registration packs when they give you your University accommodation details

A note for returning students - if you registered with us previously and haven't registered with another doctor (for example your home doctor) then you will still be registered with us and don't need to register again, but you do need to inform us of your new address (open our change of address page)

Before continuing please ensure you can answer YES to these two questions:

Are you a student (or partner or child of a student) at one of the following? (for children under 16 see note at bottom of page)

  • Barnsley College
  • College of Art & Design
  • College of Music
  • College of the Resurrection
  • Leeds College of Technology
  • Leeds Metropolitan University
  • Northern School of Contemporary Dance
  • Park Lane College
  • Trinity & All Saints
  • University of Leeds
  • York St John

QUESTION 2
 Do you / will you live in:

  • University Accommodation in Leeds?
    OR
  • Private accommodation in postal areas LS1 to LS7 or LS16 South of the Ring Road?

If you can answer yes to both questions, you can now select your method of registration with Leeds Student Medical Practice from the numbered options below.    Send us an e-mail to request a Registration Pack, which you can complete and FREEPOST back to us. Send an e-mail to lsmp.general@nhs.net stating your full name and postal address including postcode, put registration pack as the subject line. ATTENTION WE WILL ONLY POST TO UNITED KINGDOM ADDRESSES. PLEASE NOTE WE WILL NOT REPLY TO ANY MEDICAL ENQUIRIES SENT TO THIS E-MAIL ADDRESS

Come in to Reception
   Come in to our reception and ask to register. Reception is open Monday to Friday, 8.30am to 6pm. How to find us

NOTE: CHILDREN UNDER 16 need to register in our building. Please come to Reception with your parent or legal guardian, between 8.30am to 5.45pm, Monday to Friday, and ask to register
