C2 - Health and safety management

>> Describing the arrangements with the faculty, school or service for carrying out accident/incident reporting and investigation in their policy statement
>> Ensuring there is clarity in the way responsibility is delegated to schools and services.
>> Ensuring allocation of resources to enable appropriate standards
>> Consulting and involving staff and safety representatives during accident investigation process
>> Monitoring compliance to the standards in their areas of responsibility
>> Ensuring information, instruction and training is provided to all people involved in the accident reporting process including any specific information, instruction and training needs identified.
>> Ensuring all staff and students understand their responsibilities
>> Ensuring the nomination of training of employees to carry out accident reporting and investigation
>> Drawing up and actioning prioritised action plans to implement the findings of accident reports
>> Ensuring that findings of accident and incident reports are communicated to all employees and others as appropriate

>> Assist managers with procedures for accident / incident reporting and the investigation process.

>> Inform line managers of any difficulties or concerns with work practices, working environment or findings of the accident report

>> All staff and students must undergo local induction training and any further training as a result of an accident or incident.
>> All staff and students should be encouraged to report accident and incidents.
>> All accident forms including investigation forms must be held locally.
   
C5 - Risk assessment
>> Employers have a general duty to ensure the health, safety and welfare at work of employees and non employees, so far as is reasonably practicable.

>> Employers must carry out a suitable and sufficient assessment of the risk to health and safety of employees and non-employees.
>> Significant findings from all accidents and incidents including near misses must be recorded and risk assessments in particular control measures must be reviewed.
   
C6 - Exposure and health surveillance
>> Any ill health reports must be reported via the University accident reporting system, Sentinel.

C7 - Health and safety assistance, competent people and supervision
>> Faculty managers, HOS / Service in entrusting tasks to their employees:

C8 - Design, commissioning and installation
>> Refurbishment of buildings must be authorised by Estate Services, who will consult with University health and safety managers

>> Faculties, schools and services must liaise with Estate Services and the University health and safety managers in relation to the assessment of risk commencing at planning stages.

C9 - Statutory inspection and examination
>> Faculties, schools and services must ensure plant and equipment are safe and comply with statutory inspection requirements. Also make clear who has responsibility for maintenance, testing, monitoring and keeping records

C10 - Provision of equipment and maintenance
>> Identify in the risk assessment process for preventative maintenance

C11 - Personal protective equipment
>> PPE must be a last consideration in the hierarchy of control and must be fit for purpose and suited to individuals

>> Where another employer or self employed person is involved (such as in shared premises or in the use of contractors) appropriate measures need to be taken to ensure full cooperation and coordination so responsibilities are defined and all risks properly addressed

>> In situations involving other employers and the University of Leeds working together, the responsibilities of each party for managing and monitoring health and safety must be agreed and documented prior to commencement of work activities. Employers are ultimately responsible for their staff even if they work in a non-University of Leeds building or are managed by a non-University of Leeds manager
>> Any accidents or incidents involving third parties must be reported and investigated.
   
C13 - Emergency planning, contingency plans and emergency actions
>> Managers are responsible for ensuring supplementary documentation is completed in accordance with activities being assessed;

>> Information relating to a serious accident or incident that might attract media interest must be channelled through the communications team.
